I have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.

I have not, nor any members of my household, traveled by sea or by air, internationally within the past 30 days.

I have not, nor any members of my household, visit any local area within the United States that was reported to be highly affected by COVID-19, in the last 30 days.

I have not, nor any members of my household, diagnosed to be infected of COVID-19 virus within the last 30 days.

I do not believe, nor any members of my household, have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/ COVID-19.

I voluntarily seek services provided by Twentieth Capital Group LLC (Blow to Paris) and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.

I further acknowledge that Twentieth Capital Group LLC (Blow to Paris) cannot guarantee that I will not become infected with the Coronavirus/Covid-19.

I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.

I further acknowledge that Twentieth Capital Group LLC (Blow to Paris) has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. To this extent, are following the Center of Disease Control (CDC) and local health department guidelines with regard to social distancing practices in order to reduce the spread of Novel Coronavirus, or COVID-19.

I further acknowledge that Twentieth Capital Group LLC (Blow to Paris) staff is required our staff to maintain distance between themselves and you, the client, as much as possible. We will also be requiring this same procedure for client-to-client contact in order to limit the exposure to all individuals.

All individuals (staff and clients) are require to utilize either surgical masks or cloth masks to reduce the risk of exposure to yourself and others. It is also required of everyone to either wash or sanitize their hands after using the restroom, sneezing, coughing, and before eating meals.
